Users who utilize Helpdeks services are subject to the following conditions,
Waiver of Liability
I grant permission to the University of Rhode Island Help Desk Student Technicians to perform a technical assessment of my computer to diagnose and then repair any problems related to malware or the opertaing system. Remediation actions may include the removal of malware (viruses, spyware, adware, Trojans, bots, etc.) to prevent any malicious activities or technical issues on my computer system(s). Because some malware can be hiding in the computer registry, I understand that this process could possibly result in data loss, hardware failure, or additional unforeseen consequences. Recognizing that some malware is particularly difficult to find and remove, I understand that in extreme cases I may need to have my computer reformatted and the operating system reinstalled.
By signing this form I acknowledge and agree to release the URI Help Desk, and the employees of said entities from liability. This includes releasing the parent organization, the Media and Technology Services Department (the providers of networking and Internet services to URI), and the University. I understand there exists no implicit or explicit guarantee regarding repair success, system functionality or access availability to systems before, during, or after service or repairs.
If my computer is diagnosed with malware and if any person (myself, a University employee, a student technician or otherwise) decides to stop repairs and services, or decides not to proceed with this process, it is possible that my connection to the URI network, and internet connection, could be terminated in order to prevent any further security risk(s) to the University network.
ALL CLAIMS FOR LIABILITY OR LOSS INCLUDING WITHOUT LIMITATION ANY INDIRECT, INCIDENTAL OR CONSEQUENTIAL DAMAGES THAT MAY OCCUR AS A RESULT OF ANY ACTION OR INACTION BY THE UNIVERSITY OF RHODE ISLAND OR ITS EMPLOYEES ARE HEREBY EXPRESSLY WAIVED.
I agree to all of the foregoing conditions and wish to have diagnosis and remediation actions performed subject to the conditions listed.
